With FITSTATS you can easily adapt your wellness questionnaire to become a professional, app-based, COVID-19 screening system. Here is how to do it.


Step 1 - Add the COVID-19 question category

Open School Preferences – Wellness questionnaire. Rename the 'Health' wellness category to 'COVID-19' and click Save.


Step 2a – Add COVID-19 morning questions

Open Team settings - Wellness questionnaire section again, add COVID-19 questions.


1. Select the COVID-19 category

2. Morning in the Morning/Evening drop down list

3. Enter the name of COVID-19 subcategory (exposure, temperature, symptoms, etc.) in the Short code box

4. Enter No and Yes for, respectively Answer 1 and 2.


Repeat this operation for all COVID-19 related questions.

  1. Is your temperature greater than 100.4 F or 38.2C? (Temperature category)
  2. Have you been exposed to or cared for or do you live with someone who is currently ill with COVID-19, who has symptoms consistent with COVID-19 or have you been instructed by a physician to remain home from work due to any similar contact? (Exposure category)
  3. In the last 24 hours have you had symptoms consistent with COVID-19 such as chills or fever, cough, sore throat, vomiting or diarrhea, aching throughout the body, difficulty breathing or a loss of your sense of smell or taste OR have you been instructed by a physician to self-quarantine because of these or similar symptoms? (Symptoms category)

Step 2b – Add COVID-19 evening questions (optional)

Open the Wellness questionnaire section again, set the Evening questionnaire option to Activate and select the time of day at which the second questionnaire should be entered. 

Add the second set of questions (or similar), associate them with the newly created COVID-19 categories and select Evening in the Morning/Evening drop down list.

  1. Is your temperature greater than 100.4 F or 38.2C? (Temperature category)
  2. Since this morning, have you been exposed to or cared for or do you live with someone who is currently ill with COVID-19, who has symptoms consistent with COVID-19 or have you been instructed by a physician to remain home from work due to any similar contact? (Exposure category)
  3. Since this morning,  have you had symptoms consistent with COVID-19 such as chills or fever, cough, sore throat, vomiting or diarrhea, aching throughout the body, difficulty breathing or a loss of your sense of smell or taste OR have you been instructed by a physician to self-quarantine because of these or similar symptoms? (Symptoms category)

Step 2c – Add custom fields to collect morning /evening temperature (optional)

Open the Wellness questionnaire section, scroll down to Custom fields, click the small green cross to create a new field. 

Then enter Temperature in the text box, select Show, Decimal, C as unit. Also select Daily z-score (higher better) and Morning or Evening. If you need to collect temperature for Morning and Evening, create two custom fields and name them ‘Morning Temperature’ and ‘Evening Temperature’.


Step 3 - Adjust your team dashboard to display COVID-19 alerts

From School Preferences – Wellness Dashboard, deselect non-essential wellness measures and select the 3 covid-19 categories (temperature, exposure, symptoms) and click Save.

Step 4 – Set automated reminders

Open Team settings- Reminders. To enable the morning reminder, enable the Wellness questionnaire reminder button., to activate the evening reminder, activate the - Wellness questionnaire reminder (Evening). Then save. 


Morning reminder will be sent at the time set in the Time of the day section. Evening reminder will be sent at the time selected in Step 2b.


Step 5 – Activate COVID-19 Alerts (optional)

If you want participants and staff to see COVID-19 specific alerts and instructions, select the Modules-Alerts-Student button.

Then open the Alerts-Wellness and activate the COVID-19 button. If you want to edit the alert message and instructions provided to participants, click the exclamation mark and make the required changes.


Step 6 - You're done!

Athletes will now be able to answer covid-19 specific questions as part of their daily or twice a day wellness questionnaire, and you’ll be able to identify who’s at risk or not, directly from your team dashboard.


To visualize, print or export COVID-19 screening data, go to Log and Stats, or open the various Wellness graphs and reports, do as you will do for any wellness variable.
